Club Sports Committee Application
A copy of this form should be submitted within one week of travel for any trip outside of the Houston area. A form should be completed for each vehicle driven and should include all drivers and passengers present in the vehicle.

Please fill the form out and submit to the Intramural Sports Office (second floor above the gymnasium) no later than 5 p.m., Wednesday, March 26, 2008, or use the above form to apply. For more information on the role of the Committee or to obtain additional copies of this application, contact Rachel Patterson (clubsports[at]rice.edu).
When all applications are received, members of the Committee may call you to discuss your application and to answer any questions you may have regarding your involvement on the Committee. On behalf of the CSC, we thank you for your interest.
